Inquiry and Application

Initial Contact: Reach out via phone, email, or our website for day hab services.
Submit Application: Complete and submit the application form with required documents, such as medical records and/or individualizes support plans.

Assessment and Evaluation

Schedule a Meeting: Meet with our intake coordinator for an initial assessment to discuss needs and goals.
Facility Tour: Tour our facility to see our environment and meet our adult daycare team.

Enrollment and Orientation

Receive Decision: Get an acceptance notification with enrollment details.
Complete Enrollment: Sign the enrollment agreement and attend the orientation session to get acquainted with our Lancaster staff.
